Disclosing Mental Health at Work

Have you ever been diagnosed with some form of clinical depression (including major depressive disorder, bipolar disorder, dysthymia and others)? Are you currently employed? Are you interested in helping researchers better understand your experiences at work? 

If you said yes to all the previous questions, please visit the website below to fill out an online survey (approximately 30 minutes long) concerning disclosure of depression diagnosis at work.  This survey is anonymous, and we will not be able to identify you by your answers.
